What role will you be serving on this program abroad?

Program leaders should select "Program Director/Co-Director."
All UGA employees who are not directors should select "UGA Program Faculty/Staff," including Graduate Assistants.
Any person who is not a UGA employee NOT with one of the International Centers should select "Non-UGA Staff/Individuals."
All personnel with one of UGA's International Centers (Cortona or Oxford), both UGA and non-UGA employee should select "International Centers Faculty/Staff."

Note: Please have a copy of your PASSPORT ready to upload when completing the Program Agreement.

Some information requested is needed for insurance and STEP enrollments.
If a passport is not currently available, please submit this form at a later time when it is available.
If a passport is expired, please submit the most current version of it. When a new version is available, please contact studyawayhelp@uga.edu to update it.
